With MedSur One, services for palliative care, non-acute, chronic, and primary care conditions such as listed are provided to Adults and/or Seniors age 50-80 and Elderly age 81 -105.
Primary care Chronic Conditions treated along with others:
1. Uncomplicated cases of allergy/asthma, Chronic bronchitis, conjunctivitis, genitourinary conditions, low back pain, otitis media, rashes,upper respiratory infections, COPD, Type 2 diabetes,mild-moderate depression, gastric disorders,Congestive heart failure, hypertension. ESRD related services, osteoarthritis, anemia, high cholesterol
2. Consultations regarding prevention and wellness
services such as immunizations, smoking cessation,
nutrition and physical activity, wellness exams.
3. Pharmacological management and education on medications and herbal supplements patient may be taking.
4. Alcohol misuse screening, depression screening,
preventive services. Admission of patient's who have no
healthcare provider.
5. Follow-up visits, specialists referral, Therapies, labs, and imaging.
6. Evaluations for Home Health Care, Physical Therapy, Speech
Therapy, Occupational Therapy, etc. And ordering such services for patients as needed
7. Palliative care: Palliative care differs from Hospice in that Palliative care consist of building support for adults/seniors with serious illness. This consist of curative, remissive care that may focus on relief from symptoms, pain, and stress of a serious illness-whatever the diagnosis may be. All care is provided in collaboration with the patient's other care team providers.
Location of Services
With Telemedicine we are able to provide service to any
location with internet and also to anyone with a smart
phone, tablet or computer.
Services are to be provided in a secure," private", reasonably
soundproof room and have a lockable door to prevent
unexpected entry. Efforts shall be made to ensure privacy
so the provider discussion cannot be overheard by others
outside of the room where the service is provided.
Appropriate Locations
Service sites may be in patient's home, group homes, rural health clinic, library, or any area with broadband/privacy, but primarily in the home.
What is Telemedicine/Telehealth?
Telemedicine/Telehealth is the ability to remotely access, diagnose, monitor and treat patients by means of telecommunication. Basically using audio and video equipment to have live-two way, interactive communication between the provider and patient in privacy, who may not be at the same location, but on the other side of the country.
While doing house calls (home visits) for patients, I
found that my real joy came from the time I was able to
spend with each individual, offering longer visits to educate,
train and help empower them in managing their chronic
diseases. These visits increased individuals quality of life,
increased their levels of satisfaction, helped them to
feel well cared for. and helped them to live a more
productive life at home, reducing unnecessary hospital and
ER visits and nursing home admissions.
Telemedicine/Telehealth makes it easier for providers (MD's, NP's, PA's) to maintain patient's privacy, ensure long-term outcomes, reduce risk
for medical mistakes, reduce in-facility infections and
allow for appropriate time to teach patients steps in
managing their chronic conditions. Because of the time
spent with our patients, we can ensure continuity of care
and adhere to treatment guidelines.
